The Space
Diagnostics sits at the intersection of therapeutics and medical devices: the science has to be as rigorous as a drug program’s, and the path to market often runs through the same regulatory and reimbursement questions a device faces.
A diagnostic can be analytically excellent — accurate, precise, reproducible — and still fail commercially if it does not change what a clinician does next. That gap, between a test that works and a test that changes a decision, is where most diagnostics companies actually live or die.
Why It Matters
The gap between promising and investable.
Investors in this space have to separate three different questions that founders sometimes blur together: does the test measure what it claims to measure, does that measurement correlate with the clinical outcome that matters, and does a clinician or payer actually act differently because of the result.
A diagnostic that is scientifically sound but does not clear that third bar — clinical utility — struggles to build a reimbursement case no matter how strong the underlying assay is.

Is there a credible coding and coverage strategy, or is commercialization dependent on a payer decision nobody has tested?
